Estudio de caso: Discectomía anterior y fusión C3-4, C4-5

y C5-6 e Inserción del globo de la jaula de titanio intersomático;

C3-4, C4-5 y C5-6

Un paciente que comenzó a tener debilidad en la mano y problemas de destreza junto con desequilibrio durante el último mes. Tenían retención de orina con incontinencia por rebosamiento. No habían podido caminar durante un mes. No han podido usar sus manos para las actividades de la vida diaria. Sentían que habían empeorado y luego se habían estabilizado. No han tenido recuperación hasta ahora.

Se realizó una resonancia magnética y se encontró que tenía estenosis grave en C3-4 con estenosis moderada a grave en C4-5 y C5-6. El Dr. Mariwalla y yo discutimos las opciones de tratamiento y el paciente optó por el manejo quirúrgico. Discutimos la necesidad de discectomía cervical anterior y la colocación de jaulas, así como tornillos.

También discutimos con respecto a los riesgos y beneficios, así como las complicaciones que incluyen sangrado, infección, lesión de nervios y vasos adyacentes, agravamiento o empeoramiento del problema, la necesidad de atención en la UCI, la necesidad de rehabilitación, el fracaso del implante en el futuro, Necesidad de cirugías, necesidad de cirugía posterior para retroceder en caso de que la estabilidad no esté allí.

Enfermedad del segmento adyacente que involucra el proximal de los discos distales, problemas sistémicos como ceguera, parálisis, eventos cardíacos, neurológicos, pulmonares, trombosis venosa profunda, entre otros. El paciente entendió y firmó el consentimiento.

El paciente fue llevado al quirófano donde se indujo anestesia general. Se colocaron múltiples vías intravenosas. Los SSEP y los MEP fueron creados por el equipo de electrodiagnóstico. El paciente fue trasladado a la cama Jackson en posición supina. Se puso un rollo entre los hombros.

La cabeza fue puesta en una rosquilla y pegada a la cama. El cuello se preparó después de cortar los pelos asépticamente y drapeados. Se administraron antibióticos intravenosos para iniciar el procedimiento. Se pidió tiempo de espera.

Se planificó una incisión transversal en el lado derecho del cuello, centrada sobre la vértebra C4. Se utilizó el brazo C para localizar el nivel. Se realizó una incisión transversal de unos 5 cm sobre el lado derecho del cuello. Se logró la hemostasia.

Platisma fue inciso en línea con la incisión. Se realizó disección subcutánea y subplatismática. Se formó un intervalo entre el esternocleidomastoideo y los músculos de la correa. Con la disección contundente se creó usando un dedo y Kittner un avión.

La carótida podría seguirse profundamente hasta el esternocleidomastoideo lateralmente. Se alcanzó la vértebra de la línea media y se pudo ver el coli largo. Las estructuras mediales se diseccionaron medialmente usando Cloward de mano. Las estructuras laterales fueron diseccionadas lateralmente con la de mano Cloward.

El espacio del disco fue localizado y marcado con la aguja espinal. Se utilizó el brazo C para localizar el nivel y se encontró que era C4-5. Se retiró el brazo en C y se marcó el espacio del disco con el Bovie para la localización. Ahora, la disección de la línea media se realizó elevando el colli largo a cada lado desde el C3 inferior al C6 superior.

Después de una muy buena exposición, los pines Caspar se insertaron en el cuerpo de C3 y C4. La retracción se realizó utilizando los retractores de auto-retención. Skyline se utilizó para diseccionar medial y lateralmente. Una vez que se logró la reducción adecuada, el disco C3-4 se extirpó utilizando curetas y fresa redonda de 5 mm. Una vez extirpado el disco se alcanzaba el cuerpo posterior. Se vieron el PLL y el disco. Todo el disco se extrajo junto con la escisión de PLL, desde el C3 inferior hasta el C4 superior.

Las foraminotomías bilaterales se realizaron con el punzón de Kerrison 3.0. Todos los discos fueron completamente decorticados para lograr la fusión. Una vez que se realizó una foraminotomía y discectomía adecuadas, se insertó un espaciador para dimensionar el disco. Después del dimensionamiento, se insertó una jaula de titanio Globus de 8 mm llena de espaciador intersomático de titanio, llena de injerto óseo autólogo, mezclado con Trinity y DBX encontrándolo en la posición adecuada.

La atención se trasladó al espacio del disco C4-5. El pasador Caspar se insertó en el cuerpo C5 y la expansión del C4-5 se realizó con un retractor de autoretención. El horizonte se movió hacia abajo para retraerse medial y lateralmente. La discectomía se realizó de manera similar, seguida de escisión del ligamento longitudinal posterior y foraminotomías bilaterales. Después de una escisión adecuada, se realizó el dimensionamiento, se volvió a insertar el espaciador de titanio Globus con Trinity e injerto óseo autólogo y DBX.

Una vez más, se insertó un nuevo alfiler en el cuerpo C6 y se realizó una discectomía similar utilizando curetas, fresa de fósforo y punzones Kerrison. PLL también fue extirpado. Se realizaron foraminotomías bilaterales de C5-6. Se utilizó una fresa de fósforo cada vez para hacer agujeros en las placas finales para una mejor fusión.

Se insertó la jaula, nuevamente empaquetada con injerto óseo autólogo, Trinity y DBX. Después de insertar las tres jaulas, el contorno óseo se realizó utilizando fresa de fósforo y se colocó una placa de un tamaño adecuado. Se encontró que la placa era adecuada en AP y vistas laterales en una posición satisfactoria.

La placa se fijó utilizando tornillos de ángulo variable de bloqueo en el cuerpo de C3, C4, C5 y C6. Las fotos finales fueron tomadas y guardadas. La herida se lavó a fondo y se logró la hemostasia. Los retractores fueron retirados. El cierre se realizó en capas usando #2-0 Vicryl y Monocryl. Se aplicó el apósito.

Se le colocó un collarín cervical duro al paciente. No hubo incidentes de neuromonitorización durante la cirugía. El paciente fue extubado y trasladado a la unidad de recuperación en condición estable. El paciente inicialmente hizo un seguimiento 10 días después de su cirugía y luego continuó con el seguimiento 2, 4, 6, 8, semanas después de la cirugía.

I'm very happy St. Catherine's sent me to this office for treatment after I broke my collar bone! Unfortunately the break was too severe to repair on its own and I had to have it rebuilt a week after the injury. Dr. Vaksha did a great job fixing it. I'm three months out, and I have full use of the arm and shoulder. The whole office staff is very professional, accommodating, and welcoming. I hope to not need orthopedic services again, but Dr. Vaksha will be my number one choice if I or anyone in family suffers such am injury.

Dr Vaksha and Dr karkare are 2 of the best orthopedic doctors around my wife Susan had a very serious broken ankle in March of 2019 she didn't think she was going to walk again Dr vaksha did an excellent surgery on her ankle he said it was his toughest he ever did he put a lot of hardware in her ankle and told her she would be alright she made remarkable recovery thanks to his surgery as of now she regained 100 percent use of her ankle she would highly recommend him to anyone. My own experience with Dr karkare has been wonderful he takes his time with you listens to what you have to say and prescribes various treatments and is very caring I would highly recommend him to anyone I would give both doctors a 10 plus rating we are very happy with them the best.

After suffering from a severe ankle injury Dr. V was able to help me heal and return back to work completely to a job where I stand for 12 hours a day. The office is very clean and I appreciated the reminders of my appointments via phone call and through text. The patient portal made it easy for me to access all my documents including work notes. I was rear ended in an auto accident , Dr Vashka was recommended by a friend of mine .I was experiencing Back , neck , and shoulder pain . After a thorough examination and given exercises to do at home , I am feeling much better , and I ended up avoiding surgery . The staff at Complete Ortho is extremely attentive and show great care when making an appointment and are very friendly and i never waited more than 5 minutes for my appointment . It was the afternoon of Friday Sept. 24. We were in Pt. Jefferson and my wife, Mary Ann, broke her hip. We went to Mather Hospital and it was determined that she would have to have an operation to have it repaired. This would be her third time under the knife in the past year. It just so happened that we were very fortunate enough to have Dr. Karkare, who was on standby, perform the surgery. He put in a rod and two screws in her hip. She spent a few days in the hospital and then went to Gurwin rehabilitee for another few weeks.It has now been almost six weeks and we both worked the election the other day. If it wasn’t for Dr. Karkare’s expertise she never would have been able to work. She is able to walk with a walker and is doing physical therapy three times a week.We can not thank the doctor enough for the compassion and dedication that he puts into his work. It allows Mary Ann do the things that she likes to do, even on a limited basis for a while. In the year of 2018 I was referred to Dr. Karkare because I was experiencing severe knee joint pain. After exhausting physical therapy and trying to labor through the pain, I had to make a quality of life decision. Total knee replacement was the only viable option. Dr. Karkare made my decision easy as he walked me through the whole process from surgery to recovery.On 12/13/19 ( Friday the 13th) I enter Lenox Hill Hospital in great hands. From the time I entered Dr. Karkare’s office for the first time until now, his staff has been amazing. Andrea the medical coordinator walked me through all the paper work and necessary preparations for the surgery. Courtesy and kind would be an understatement. I suffered with pain in both knees for years. My orthopedic doctor kept recommending knee replacement . I fought it for years, as I was just afraid. When I had no choice and could barely walk , it was recommended I see Dr. Karkare. We set up a consultation and my wife and I left his office feeling totally confident and comfortable with moving ahead with the surgery. He explained everything to us, and the office staff set everything up for us and made the process easy. So about one month after our initial meeting I had the first knee done. I was up walking mere hours after the surgery, and on the workout machines the next morning. I went home two days after the surgery, and yes walked my daughter down the aisle at her wedding only one week after the surgery without even a cane! Three months later I had the other knee done and went home the very next day. Dr. Karkare put my fears to rest . I would highly recommend him. His expertise gave me my life back. There is no better Orthopedic doctor you will find. Broke my ankle three places on a Saturday. Called Dr. Karkare. He had is team ready at the hospital and operated on me within 6 hours after my injury. Now After 3 months of great care by him and his staff, I am walking to normalcy.
