Muchos de nosotros hemos pasado por dolor de espalda y nos damos cuenta de lo perjudicial que puede ser. Hemos visto a amigos y familiares luchar con problemas de espalda a veces durante años. Para aquellos de nosotros que nunca hemos tenido dolor de espalda debemos estar agradecidos.

El dolor de espalda es el resultado de una serie de cosas que van mal con nuestro cuerpo. Los músculos que controlan las múltiples articulaciones de la columna vertebral y la pelvis se vuelven demasiado débiles para controlar el movimiento de las articulaciones o demasiado tensos para permitir excursiones normales de las articulaciones que están diseñadas para moverse.

Esto hace que las cargas que atraviesan la columna vertebral aumenten hasta el punto en que los ligamentos se inflaman, los discos se hernian, se desarrollan espolones y los nervios se ven afectados. ¿Qué puedes hacer al respecto?

Primero reduzca su actividad. El dolor de espalda significa que le está pidiendo a su espalda que haga algo que no puede. Así que pasa menos tiempo erguido y soportando peso. Trate de reducir a un nivel en el que no le duela la espalda.

Ahora que ha encontrado una actividad que su espalda puede manejar, el siguiente paso es aumentar la capacidad de la espalda para que pueda manejar más. Usted hace esto con un programa de ejercicios apropiado que involucra fortalecimiento y resistencia.

El fortalecimiento debe centrarse en los músculos de la espalda y el abdomen deben hacerse en posición supina. Los ejercicios de resistencia deben hacerse semi o sin soporte de peso, ya sea en una bicicleta o en una piscina.

Sentarse tiende a empeorar los problemas de espalda. Pruebe con un rollo de toalla en la parte baja de su espalda. Levántate cada 10 minutos y estira la espalda. Considere una silla arrodillada.

Recordar. El dolor significa que su espalda no puede hacer lo que le está pidiendo que haga.

Si intenta tomar analgésicos y sigue haciendo lo que está haciendo, entonces su dolor empeorará. El remedio para su dolor se vuelve más prolongado e involucrado.

El dolor de espalda significa que no puedes hacerlo. Atrás.

Comience un programa de ejercicios. Los analgésicos no curan la espalda, así que no actúes como si lo hicieran.

ANATOMÍA DE LA COLUMNA VERTEBRAL

La columna vertebral es una estructura compleja que está hecha de múltiples huesos llamados vértebras junto con sus ligamentos, cápsulas, músculos y estructuras nerviosas asociadas. Se extiende desde la base del cráneo hasta la pelvis. La columna vertebral se compone esencialmente de 33 vértebras. Las cuatro vértebras inferiores se unen para formar el cóccix o el coxis.

Las cinco vértebras por encima del cóccix se unen para formar el sacro. El resto de las otras vértebras son unidades móviles. La columna vertebral se divide esencialmente en 3 partes. El cuello o la columna cervical, la espalda media o la columna torácica, y la espalda baja o la columna lumbosacra.

La columna cervical está formada por siete vértebras superiores. Permite el movimiento de la cabeza. La columna torácica está formada por doce vértebras y bisagras de las costillas en la parte delantera para hacer el pecho. La columna lumbosacra se compone de cinco vértebras lumbares, cinco sacras y cuatro del cóccix y ayuda a distribuir el peso de la parte superior del cuerpo a la pelvis y las extremidades inferiores.

ANATOMÍA DE LAS VÉRTEBRAS DE LA COLUMNA VERTEBRAL

La vértebra es la unidad básica de la columna vertebral. Cada vértebra se compone de dos partes, la parte sólida frontal llamada cuerpo, que es fuerte y estable y se apilan una sobre la otra para proporcionar estabilidad a la columna vertebral. La parte posterior de las vértebras está hecha de un hueso plano en forma de V llamado lámina y está unida al cuerpo por dos pedículos.

La lámina forma el arco sobre la parte posterior del cuerpo para formar un canal a través del cual la médula espinal y las raíces nerviosas pasan y están protegidas. Hay agujeros vertebrales debajo de cada pedículo a través de los cuales la raíz nerviosa del nivel específico sale del canal espinal para suministrar las estructuras en sus respectivas áreas.

La porción de la unión de la lámina con el pedículo se llama pars interarticularis. Esto puede estar involucrado o fracturado en algunos pacientes (especialmente pacientes jóvenes) causando inestabilidad de la columna vertebral. Hay procesos articulares por encima y por debajo de la parte posterior de la lámina llamados facetas que se articulan con las facetas por encima y por debajo en cada lado para formar las articulaciones facetarias. Los dos cuerpos vertebrales se articulan entre sí también a través de un disco intervertebral intermedio.

DISCO INTERVERTEBRAL

El disco intervertebral está presente entre los cuerpos de las vértebras por encima y por debajo. Ayuda a proporcionar estabilidad y movilidad a la columna vertebral. También actúa como amortiguador previniendo lesiones en el cuerpo vertebral. Un disco está formado por anillos fibrosos externos llamados anillo fibroso y la gelatina interna llamada núcleo pulposo.

El disco puede dañarse debido a una lesión, desgaste o envejecimiento. Esto puede conducir a la hernia del núcleo a través del anillo fibroso causando hernia de disco. Esta hernia de disco puede causar compresión sobre la médula espinal o las raíces nerviosas, lo que puede provocar dolor radicular en la pierna o el brazo llamado radiculopatía. La degeneración del disco en sí puede causar dolor de espalda.

FUNCIÓN DE LA COLUMNA VERTEBRAL

La columna vertebral funciona para mantener nuestro cuerpo erecto y recto, así como proteger todos los nervios en forma de médula espinal y las raíces nerviosas que provienen del cerebro dentro del canal de la espina y se distribuyen por todo el cuerpo.

Los segmentos móviles de la columna vertebral en la columna cervical, torácica y lumbar están hechos esencialmente de tres articulaciones entre cada vértebra que permiten el movimiento y la estabilidad.

La columna vertebral está envuelta con músculos, especialmente en la espalda y en sus lados. Los vasos principales del cuerpo corren a lo largo de la parte frontal de la columna vertebral en la columna torácica y lumbar, mientras que la comida y la tráquea corren frente de la columna cervical con otras estructuras complejas y vitales alrededor de la columna vertebral.

ENFERMEDADES DEL ESPINA

Debido a la complejidad de sí misma y sus estructuras circundantes, la columna vertebral es propensa a muchos tipos de enfermedades. Las enfermedades que generalmente afectan la columna vertebral son cualquiera de los siguientes tipos:

  • Congénitas o del desarrollo: este tipo de afecciones generalmente están presentes al nacer o se desarrollan en los primeros años de desarrollo de la vida. El desarrollo de la escoliosis al nacer o durante la adolescencia es un ejemplo de este tipo de problema.
  • Lesión traumática de la columna vertebral – Estas lesiones ocurren ya sea debido a un accidente automovilístico o caídas u otros accidentes similares. Los pacientes pueden sufrir una fractura o una luxación o una combinación de ambas. Tales pacientes deben ser tratados en consecuencia bajo la supervisión de un cirujano de columna vertebral.
  • Neoplásico – Significa que el paciente tiene una lesión neoplásica o tumor que afecta la columna vertebral. Estas lesiones pueden ser malignas o benignas y, por lo general, necesitan un estudio de diagnóstico para descartar o confirmar el diagnóstico antes de que se pueda planificar y llevar a cabo un tratamiento.
  • Infeccioso – Significa que la columna vertebral está involucrada en una infección. La infección de la columna vertebral es un fenómeno poco común, pero puede ocurrir especialmente en pacientes inmunocomprometidos como pacientes de VIH o SIDA o pacientes que están en quimioterapia o radioterapia o en medicamentos inmunosupresores. También es común en usuarios de drogas intravenosas.
  • Enfermedades metabólicas e inflamatorias de la columna vertebral – Estos generalmente incluyen osteoporosis u osteomalacia, lo que significa debilitamiento de la estructura ósea. Esto generalmente ocurre debido al envejecimiento, pero también puede ocurrir debido a medicamentos o enfermedades. La enfermedad inflamatoria más común de la columna vertebral es la artritis reumatoide que generalmente involucra la columna cervical o el cuello.
  • Degenerativo – Esta es la causa más común de enfermedades de la columna vertebral, generalmente involucra la columna cervical o lumbar. Esencialmente implica el proceso de envejecimiento, pero puede acelerarse debido a lesiones o al uso excesivo de la columna vertebral o a una mala postura durante mucho tiempo. La mayoría de los pacientes suelen presentar dolor en la zona afectada, ya sea en el cuello o en la parte baja de la espalda en la mayoría de los casos.

PRESENTACIÓN DEL PACIENTE CON PROBLEMA DE COLUMNA (SÍNTOMAS)

La presentación más común de un paciente con problemas de columna vertebral es el dolor localizado en el cuello o la espalda. El dolor puede estar en la línea media junto con en la ubicación paramediana. Puede estar asociado con espasmos musculares.

El dolor generalmente se agrava con la actividad y se alivia con el descanso. Ocasionalmente, el paciente puede tener dolor irradiado hacia abajo de la extremidad con o sin hormigueo, entumecimiento, debilidad de los músculos, afectación del intestino o la vejiga o la marcha. El intestino y la vejiga pueden estar en forma de retención o incontinencia.

El paciente también puede tener disfunción sexual raramente. En raras ocasiones, en casos de enfermedad de la columna torácica, puede haber dolor irradiado u hormigueo y entumecimiento a lo largo del pecho o el abdomen.

BANDERAS ROJAS

  • Los pacientes que tienen dolor de espalda o cuello con cualquiera de estas siguientes condiciones deben buscar atención médica tan pronto como sea posible.
  • Cáncer, neoplasia o neoplasia maligna.
  • Pérdida de peso inexplicable de más de 10 libras en los últimos 6 meses.
  • Comorbilidades inmunocomprometidas como quimioterapia o radioterapia o tener enfermedades como VIH o SIDA o estar en terapia prolongada con corticosteroides.
  • Usuarios de drogas intravenosas.
  • Infección del tracto urinario.
  • Fiebre de más de 100 grados centígrados.
  • Trauma significativo por una caída o accidente.
  • Afectación intestinal o vesical en forma de incontinencia o retención.
  • Debilidad en la articulación principal del brazo o la pierna.

SALUD DEL ESPINA

Nuestra columna vertebral tiene forma de S y curva que se curva hacia adelante en el cuello y la espalda baja mientras se curva hacia la espalda en la columna torácica. Una columna vertebral sana con una buena postura y una musculatura fuerte es la clave para la prevención de problemas de la columna vertebral. Hay múltiples cambios en el estilo de vida que se pueden hacer para prevenir el desarrollo de problemas de la columna vertebral, así como prevenir el deterioro de enfermedades preexistentes.

Los ejercicios regulares que también incluyen ejercicios de fortalecimiento del núcleo que ayudan a fortalecer los músculos en la parte delantera y posterior de la columna vertebral son útiles en la prevención de la enfermedad de la columna vertebral. Estos ejercicios se pueden realizar en casa o en el gimnasio con o sin la ayuda de un preparador físico. El yoga es una de las muchas buenas rutinas de ejercicios que pueden ayudar a estirar y fortalecer todos los músculos de la espalda, aparte de los músculos de las extremidades.

I'm very happy St. Catherine's sent me to this office for treatment after I broke my collar bone! Unfortunately the break was too severe to repair on its own and I had to have it rebuilt a week after the injury. Dr. Vaksha did a great job fixing it. I'm three months out, and I have full use of the arm and shoulder. The whole office staff is very professional, accommodating, and welcoming. Dr Vaksha and Dr karkare are 2 of the best orthopedic doctors around my wife Susan had a very serious broken ankle in March of 2019 she didn't think she was going to walk again Dr vaksha did an excellent surgery on her ankle he said it was his toughest he ever did he put a lot of hardware in her ankle and told her she would be alright she made remarkable recovery thanks to his surgery as of now she regained 100 percent use of her ankle she would highly recommend him to anyone. After suffering from a severe ankle injury Dr. V was able to help me heal and return back to work completely to a job where I stand for 12 hours a day. I was rear ended in an auto accident , Dr Vashka was recommended by a friend of mine .I was experiencing Back , neck , and shoulder pain . After a thorough examination and given exercises to do at home , I am feeling much better , and I ended up avoiding surgery . The staff at Complete Ortho is extremely attentive and show great care when making an appointment and are very friendly and i never waited more than 5 minutes for my appointment . It was the afternoon of Friday Sept. 24. We were in Pt. Jefferson and my wife, Mary Ann, broke her hip. We went to Mather Hospital and it was determined that she would have to have an operation to have it repaired. This would be her third time under the knife in the past year. It just so happened that we were very fortunate enough to have Dr. Karkare, who was on standby, perform the surgery. He put in a rod and two screws in her hip. She spent a few days in the hospital and then went to Gurwin rehabilitee for another few weeks.It has now been almost six weeks and we both worked the election the other day. If it wasn’t for Dr. Karkare’s expertise she never would have been able to work. She is able to walk with a walker and is doing physical therapy three times a week.We can not thank the doctor enough for the compassion and dedication that he puts into his work. It allows Mary Ann do the things that she likes to do, even on a limited basis for a while. In the year of 2018 I was referred to Dr. Karkare because I was experiencing severe knee joint pain. After exhausting physical therapy and trying to labor through the pain, I had to make a quality of life decision. Total knee replacement was the only viable option. Dr. Karkare made my decision easy as he walked me through the whole process from surgery to recovery.On 12/13/19 ( Friday the 13th) I enter Lenox Hill Hospital in great hands. From the time I entered Dr. Karkare’s office for the first time until now, his staff has been amazing. Andrea the medical coordinator walked me through all the paper work and necessary preparations for the surgery. Courtesy and kind would be an understatement. I suffered with pain in both knees for years. My orthopedic doctor kept recommending knee replacement . I fought it for years, as I was just afraid. When I had no choice and could barely walk , it was recommended I see Dr. Karkare. We set up a consultation and my wife and I left his office feeling totally confident and comfortable with moving ahead with the surgery. He explained everything to us, and the office staff set everything up for us and made the process easy. So about one month after our initial meeting I had the first knee done. I was up walking mere hours after the surgery, and on the workout machines the next morning. I went home two days after the surgery, and yes walked my daughter down the aisle at her wedding only one week after the surgery without even a cane! Three months later I had the other knee done and went home the very next day. Dr. Karkare put my fears to rest . I would highly recommend him. His expertise gave me my life back. There is no better Orthopedic doctor you will find. Broke my ankle three places on a Saturday. Called Dr. Karkare. He had is team ready at the hospital and operated on me within 6 hours after my injury. Now After 3 months of great care by him and his staff, I am walking to normalcy.
